New York State requires physical examinations for all students entering Kindergarten, as well as Grades 2,4,7,10 and all new students.  Physicals are also required for any student who wishes to participate in our Modified, Junior Varsity or Varsity sports program.  Physical exams are offered at various times throughout the year, in school, at no cost to our students. You may choose to have these completed by your personal health care provider, but we prefer you use our school forms so no information is excluded, since the forms must be approved by our school health care provider before your child may participate in any practice or sporting event.  Physicals are considered current if they have been performed within one year of the required date , however for a sports physical, must be in effect for the entire season for a student to be eligible to play.

Summer Sports Physical - Schedule  

Sports Physicals will be given throughout the summer, to enable our students to be ready to play for the Fall season.  

Due to our construction project, ALL physicals will be given at the HIGH SCHOOL, in the Nurse’s office. (Mexico Middle School will be closed this summer, student's medical records will be available at the HS Nurse's Office).

On Monday, June 24th the parents can start calling the Main Office (963-8400 Ext. 5118 - Kathy Yost) to schedule summer physical appointments, students must sign up in a time slot....no walk-ins are allowed.  We will schedule JV and Varsity student athletes first.  If it is a modified sport then we will fill the later August dates with those students. ** Pop Warner Football and Cheerleading physicals are NOT done during our summer offerings for physicals.

Parents can get the physical forms outside of the nurse's office at the HIGH SCHOOL and MIDDLE SCHOOL and also on our school website. (the health history form MUST be completed PRIOR to a student getting a sports physical) AGAIN... THE HEALTH HISTORY FORM MUST BE COMPLETED AND SIGNED BY A PARENT WHEN A STUDENT ATHLETE COMES FOR THEIR SCHEDULED PHYSICAL.

Sports physicals must be approved by our medical director prior to a student participating in a sport.. therefore .. if a student hands in a physical to a coach/nurse the day of practice, they still will NOT be able to participate until that physical is approved.  (this may take a few days, as these are approved through he Pulaski Health Center).

A sports physical is valid for one year and must be valid throughout the entire sports season... this means for falls sports, the student would need a physical dated 11/1/2012 (nothing prior) to be eligible to participate in Fall sports.
